Well-defined heterogeneous mass is seen involving the Hoffa's fat pad. This mass demonstrates a heterogenous predominant low signal intensity on both T1 and fluid-sensitive images. The mass causes a focal erosion of the tibia and measures 6.2 x 4.3 x 3.3 cm in its maximum TS, AP and CC diameters.
Case Discussion
MRI findings are consistent with localized tenosynovial giant cell tumor involving Hoffa's fat pad. Tenosynovial giant cell tumors are a group of benign neoplasms that may arise from synovium of joints, bursae and tendon sheaths. TGCT usually affect the tendon sheaths of the fingers, however it may involve different joints, such as the wrist, knee, ankle and foot. These lesions may be localized or diffuse, and on MRI it usually elicit heterogenous low signal intensities on T1- and T2-WIs.
