Horizontal tear and flap tear of the medial meniscus

A horizontal tear of the posterior horn and body of the medial meniscus is seen, extending to the tibial surface. Slightly more anteriorly a flap tear is visible, which is displaced into the superior parameniscal recess, between the medial femoral condyle and the deep part of the MCL; the free edge of the medial meniscal body at this location is blunted as a result of the tear.
Case Discussion
Horizontal meniscal tears are usually degenerative in nature and often asymptomatic. On the contrary, any displaced tear, also arising from a horizontal tear, is very likely to cause pain.
It is important to report flap tears and their displacement site as they can be missed at arthroscopy.
