Perianal pain and discharge.
Patient Data
Intersphincteric horseshoe abscess measuring approximately 13 mm in maximum thickness, extending from about 10 - 3 o’clock position and approximately 24 mm superior to anal verge.
It is seen to compress and bulge into the internal anal sphincter at right posterolateral aspect with a possible internal opening at approximately 10 o’clock position.
A linear tract, measuring about 13 mm in length is seen arising from this collection at approx. 6 o’clock position. It courses anteriorly, superiorly and laterally along the internal anal sphincter with internal opening at 3 o’clock position. Mild inferior extension of this tract is also seen along the internal sphincter for approx. 4 mm.
Edema and hyperintensity is seen with intersphincteric plane and along the internal sphincter.
Case Discussion
Findings consistent with a horseshoe intersphincteric abscess and fistula.
