On the scout image, there is a soft tissue mass on either side of the midline with a central isthmus, following contrast injection the orientation of the pelvicalyceal system is clearly outlined. Lower pole of kidneys are closest to midline, which is the reverse of normal. Both kidneys appear rotated and lower poles are fused with formation of isthmus at the level of L3-L4.
