Huge intramural fibroid

Case contributed by Hidayatullah Hamidi
Diagnosis almost certain

Lower abdominal pain and vaginal discharge.

Patient Data

Age: 30 years
Gender: Female

Two (A and B) large, well defined, intramural lesions displacing the endometrial cavity (open arrows) toward the right side and posteriorly.

The lesions demonstrate low signal intensity on T1WI and high signal intensity on T2WI. Heterogeneous post contrast enhancement is noted with non-enhancing areas (cystic degeneration).

No adjacent infiltrative/inflammatory changes. No enlarged pelvic lymph nodes. No free fluid in the pelvis.

Case Discussion

Uterine fibroids (leiomyomas) are common, benign uterine lesions. They are usually detected incidentally but if large enough, they can present as palpable masses.
