Huge subserosal uterine leiomyoma

Case contributed by Dr Ammar Haouimi

Painless pelvic mass.

Patient Data

Gender: Female

There is a huge subserosal uterine leiomyoma (13x10x7 cm), arising from the anterior wall, of intermediate signal intensity on T1WI, low signal intensity on T2WI with moderate and heterogeneous enhancement following IV contrast.

Thickening of the posterior corporeal junctional zone (18 mm) suggestive of focal adenomyosis.

A small lesion is noted within the uterine cavity (14 mm) of intermediate signal intensity on T1WI and T2WI with mild enhancement most likely a submucosal leiomyoma (atypical for endometrial polyp).

Both ovaries appear normal with minimal effusion seen in Douglas pouch.

Case Discussion

MRI features of a huge subserosal uterine leiomyoma with associated focal adenomyosis.
