Humeral greater tuberosity avulsion fracture at supraspinatus attachment

Presentation

History of right shoulder trauma 4 months ago, shoulder pain since then.

Patient Data

Age: 20 years
Gender: Male

The cortical bone of the greater tuberosity of right humerus is partially avulsed at the attachment of the supraspinatus tendon. The underlying altered bone marrow shows low signal in T1 and high signal in T2/PD FS sequences. 

The supraspinatus tendon shows abnormal high signal in T2/PD FS at its insertion point. 

The complementary radiography shows cortical irregularity superior to the humeral greater tuberosity.

Case Discussion

Regarding the MR and radiographic features, partial avulsion of the right humeral greater tuberosity at supraspinatus tendon attachment with its partial thickness tear are evident.
