Case Discussion

Low volume lungs in a preterm neonate must make you think of hyaline membrane disease (RDS).  Often treatment precedes the radiograph and appearances may be significantly less dramatic than this case.

Treatment with surfactant down an endotracheal tube may result in patchy resolution depending on the timing of follow up radiographs.
