Presentation

Pelvic discomfort with frequent urination. History of surgery for hepatic cyst 10 years prior.

Patient Data

Age: 50 years
Gender: Male

Large well-defined pelvic multiloculated cystic mass of intraperitoneal location in the rectovesical pouch, compressing and displacing the urinary bladder. It elicits a low signal on T1 and a high signal on T2 with no enhancement on postcontrast sequences. No evidence of fatty components or mural calcification.

Case Discussion

The MRI features of a multiloculated pelvic cystic mass in a patient with a previous history of surgery for a hepatic cyst are highly suggestive of a hydatid cyst, which was confirmed at surgery.
