Hydatid cysts of the abdominal wall and pelvis

Presentation

Painless swelling of the lower anterior abdominal wall progressively increasing in size in a patient of rural origin. The ultrasound (not shown) demonstrated complex cystic masses.

Patient Data

Age: 50 years
Gender: Female
  • multivesicular cystic masses of the anterior abdominal wall involving the rectus abdominis muscles mainly on the right, protruding into the pelvis. They elicit a low signal on T1, high signal on T2 with no restricted diffusion or enhancement on postcontrast sequences
  • multivesicular mass of subaponeurotic location in the right iliac fossa showing the same signal intensity as the abdominal wall masses 
  • numerous multivesicular masses in the right pelvic region of laterouterine location and in Douglas pouch with no visibility of the right ovary most likely involved. The left ovary is well--visualised on T2 and DWI
  • no other cystic mass of the liver, spleen, pancreas or kidneys

Case Discussion

The clinical presentation and the MRI features are most consistent with multiple hydatid cysts of the anterior abdominal wall and pelvis.
