Hypertrophic cardiomyopathy - systolic anterior motion of mitral valve

Presentation

Known cardiac disease. Screening for potential surgery.

Patient Data

Age: 55 years
Gender: Female

There is an impressive degree of hypertrophy involving nearly all segments except the septoapical and apical aspects of the heart. Systolic anterior motion of the mitral leaflet is seen due to the Venturi effect. Whether this leaflet is elongated or not cannot be assessed due to flow void artefact. Patchy mid-wall enhancements are consistent with fibrosis. There is no thrombus.

Case Discussion

Left ventricular outflow tract obstruction leads to a more aggressive treatment in cases of hypertrophic cardiomyopathy, whether by surgical myomectomy or alcohol septal ablation.
