Presentation

Delayed cry with history of ventilation

Patient Data

Age: 4 days
Gender: Male
mri

Diffuse cerebral edema is seen showing extensive diffusion restriction involving bilateral cerebral hemispheres including basal ganglia, thalami, corpus callosum and possibly corticospinal tracts. Mass effect is seen with compression of bilateral lateral and third ventricles.

Few microhemorrhages are seen in subcortical white matter of right frontal, left parietooccipital lobes and in right parietal periventricular white matter.

Diffusion was repeated due to some motion artifacts.

Case Discussion

Patient was found to have a sluggish pupillary reflex, no spontaneous body movements and generalized hypotonia. In the given clinical context, the imaging findings are suggestive of hypoxic encephalopathy.
