Hypoxic ischemic encephalopathy

Diagnosis almost certain

Presentation

Suicide attempt by hanging, presented with altered consciousness since 4 days.

Patient Data

Age: 13 years
Gender: Male

Diffuse supratentorial cortex, basal ganglia and thalami swelling show hypersignal on T2 and FLAIR with diffusion restriction, sparing the white matter with increased grey-white matter contrast. Effacement of the basal cisterns.

Bilateral flattening of the posterior sclera with prominent perioptic subarachnoid spaces, tortuous optic nerves, and mild tonsillar herniation consistent with increased intracranial pressure. Soft tissue edema involving the cervical, occipital and left parietal regions due to external, mechanical pressure (hanging).

MRA is unremarkable.

Case Discussion

The clinical presentation and MRI findings are consistent with global hypoxic-ischemic injury.

Unfortunately, the patient died three days later.
