Abnormal signal intensity is detected at the periventricular deep white matter (coronal radiata and centrum semiovale) bilaterally dispalying bright signal in T2 and FLAIR and low signal in T1 and T1 inversion recovery. There is no mass effect and with sparing of the subcortical U fibers, corpus callosum and brain stem.
Case Discussion
Cerebral hypoperfusion injury usually affects the watershed zones and should be differentiated from other white matter diseases e.g. leukodystrophies and congenital infection.
