Iatrogenic bile duct injury with bile leak

Presentation

History of recent cholecystectomy, now with fever and abdominal pain.

Patient Data

Age: 50 years
Gender: Male

Bile duct injury at the common hepatic duct confluence with bile leak tracking into the gallbladder fossa and then laterally along the lateral hepatic surface and the right paracolic gutter. The proximal segment of the CBD just distal to the common hepatic duct confluence is not visualized (likely ligated).

Mild proximal intra-hepatic biliary radicle dilatation.

The distal common bile duct is collapsed. The pancreatic duct is not dilated.
