Iatrogenic oesophgeal intramural hematoma

Diagnosis probable

Presentation

Recent difficult intubation (anaethetist used bougie). Developed signficant CO2 retention which dramatically improved with neck extension.

Patient Data

Age: 55 years
Gender: Female

Newly developed mediastinal mass (not present on CT 3 months earlier), which is seen as an elongated well-circumscribed non-enhancing space-occupying lesion with an average density of 50 HU, displacing the NG tube anteriorly to the left and compressing the trachea and carina. Mild bilateral pleural effusion.

Imaging appearance consistent with an esophageal intramural hematoma.

Case Discussion

Endoscopically confirmed as above. Recent 10 days interval scan showed mild reduction in size of the said hematoma.
