Iatrogenic pneumoperitoneum

Cough, chest discomfort and breathlessness, ?pneumonia

Patient Data

Age: 70 years
Gender: Male

AP chest

x-ray

Gas under the diaphragm: continuous diaphragm sign, indicative of pneumoperitoneum. This patient was one day status post laparoscopic cholecystectomy. 

Lungs and pleural spaces clear. No cardiac enlargement.

Case Discussion

Free peritoneal gas is acceptable up to 48 hours status post laparoscopic surgery. In this case of course the gas will actually be carbon dioxide and not air.
