Iatrogenic ureteric injury

Presentation

Day 5 post hysterectomy and salpingoophrectomy. Discharged following day with no complications. Admitted with sudden onset lower abdominal pain. Raised inflammatory markers ?ureteric injury ?collection

Patient Data

Age: 60 years
Gender: Female
ct

There is extravasation of urine/contrast from left distal ureter, filling the peritoneal cavity as free fluid and also a adjacent small urinoma on delayed phase images.

Mild left sided hydroureteronephrosis is noted.

Urinary bladder appears normal except for small non-dependent air within.

Right ureter appears normal.

Fluoroscopy

Retrograde ureterography showed contrast leak at left distal ureter with urinoma formation.

Wire passed to cross the injury and a stent was placed.

Case Discussion

Distal ureteric injuries are most often due to iatrogenic injury.

Surgical correction is required.

Ureteric stent is placed, if there is obstruction.
