ICA aneurysm

Case contributed by A.Prof Frank Gaillard

50 year old male with sudden collapse.

Patient Data

Age: 50
Gender: Male
Modality: CT

Intraparenchymal haemorrhage with lower attenuating "filling defect". Haemorrhage extends into ventricular system as well as into subarachnoid space. Minor hydrocephalus.

Modality: CT

EVD has been placed. CTA demonstrates a terminal ICA aneurysm.

Modality: DSA (angiography)

Large ICA aneurysm coiled.

Case Discussion

Initial CT demonstrates intraparenchymal, subarachnoid and intraventricular blood, with hydrocephalus

Within the hematoma, a rounded lower density area is present which is suspicious for a large aneurysm.

CTA (not shown) confirmed the presence of an aneurysm which was further imaged with angiography.

The aneurysm was filled with numerous coils.
