Presentation

Sudden collapse.

Patient Data

Age: 50 years
Gender: Male

Intraparenchymal hemorrhage with lower attenuating "filling defect". Hemorrhage extends into ventricular system as well as into subarachnoid space. Minor hydrocephalus.

EVD has been placed. CTA demonstrates a terminal ICA aneurysm.

Large ICA aneurysm coiled. 

Case Discussion

Initial CT demonstrates intraparenchymal, subarachnoid and intraventricular blood, with hydrocephalus

Within the hematoma, a rounded lower density area is present which is suspicious for a large aneurysm. 

CTA (not shown) confirmed the presence of an aneurysm which was further imaged with angiography. 

The aneurysm was filled with numerous coils.
