Idiopathic intracranial hypertension (pseudotumor cerebri)

Diagnosis almost certain

Presentation

5 years old child presenting with headache, blurred vision and diminished left eye vision. Fundal examination revealed bilateral papilledema.

Patient Data

Age: 5 years
Gender: Male

  • Mild ectasia and tortousity of both optic nerves as well as distended peri-optic nerve sheaths with enlarged perioptic subarachnoid CSF spaces.
  • Mild indentation of the posterior eye globe related to the optic nerve head is observed.

  • Partial empty sella.
  • Slit-likle ventricles.
  • Otherwise, normal supra- and infra-tentorial brain with no mass lesion or signal abnormality.

Case Discussion

The above described findings are consistent with idiopathic intracranial hypertension or "pseudotumor cerebri" in the appropriate clinical setting.
