IgA vasculitis - bowel findings

Diagnosis almost certain

Presentation

Currant jelly stools and abdominal pain. Concern for intussusception.

Patient Data

Age: 2 years
Gender: Male
ultrasound

There is a focally thickened loop of bowel in the left upper quadrant without definite bowel in bowel. There is hyperemia of the bowel wall. No evidence of an ileocolic intussusception.

Evaluation of the remaining four abdominal quadrants demonstrate scattered areas of fluid-filled small bowel, without significant bowel wall thickening.

Case Discussion

This is a case of the bowel findings of a patient with IgA vasculitis (also known as Henoch-Schönlein purpura), without evidence of intussusception. Pediatric rheumatology was consulted and recommended IVIG and methylprednisolone. Following several days of therapy, the patients symptoms improved.
