Ileocecal lymphoma

Presentation

Acute right lower quadrant pain. No surgical history.

Patient Data

Age: 20 years
Gender: Male
ct

Large luminal mass in the cecum, appendix and terminal ileum. The mass is relatively homogeneous with few scattered central calcifications. Mildly dilated small bowel proximal to the mass. Adjacent large, conglomerate mesenteric lymph nodes.

Case Discussion

Biopsy results showed diffuse large B-cell lymphoma. Other considerations include adenocarcinoma and carcinoid. Large tumor size and modest obstruction, along with patient demographic, favor lymphoma over adenocarcinoma. Absent desmoplastic reaction and conglomerate mesenteric lymph nodes favor lymphoma over carcinoid.
