Iliac arterial embolisation by a bullet

Case contributed by Dr Jose Miguel Terrazas Najera

Young man wounded by a firearm aimed at thorax.

Patient Data

Age: 29 years old
Gender: Male
DSA (angiography)

Entry wound was found at the level of fourth left intercostal space, without an exit wound.  The patient subsequently presented with signs of ischemia in his right lower limb.

Intraoperative angiography shows the head of the projectile embolized to the right iliac artery.

Case Discussion

Embolisation of bullet fragments to body regions distant to its initial trajectory is an uncommon, but well documented complication of gun shot injuries.
