Patent abdominal aorta and common iliac arteries bilaterally.
The left external iliac artery is patent but there is a mural irregularity, particularly affecting the lateral margin of its middle third as it lies in contact with the underlying psoas muscle.

Angiographic findings are in keeping with appearances on the MRA. There is moderate mural irregularity affecting the middle third of the left external iliac artery.
Case Discussion
The clinical presentation and radiological appearances are consistent with iliac artery endofibrosis. The patient was offered a laparoscopic external iliac endarterectomy but declined as he currently wants to manage his symptoms conservatively.
