Recurrent gingivitis at lower left posterior teeth region.

Patient Data

Age: 35 years
Gender: Male

Orthopantomography (OPG)

x-ray

Unerupted horizontally impacted left lower third molar tooth (17). It doesn't reach the inferior alveolar nerve canal.

x-ray

Unerupted horizontally left lower third molar tooth (17) impacted against the posterior root of the left second molar tooth (18) with erosion of lamina dura and resorption at the level of the tooth neck.

There is also resorption and lamina dura erosion of the posterior part of the left lower first molar tooth (19) at the level of the tooth neck.

Case Discussion

Horizontal impaction of the wisdom tooth is one of the types of impaction where the wisdom tooth lies horizontally within the mandible facing the neighboring tooth. It can present with complications like persistent pain, overlying gingivitis (pericoronitis), and caries of neighboring teeth which all happen in this case. Treatment of symptomatic impacted wisdom tooth would be extraction and repair of local complications. Teeth decay is more visualized in local x-ray than orthopantomography and would be even better on cone beam CT scan.
