Presentation
History of hypertension, hypersensitivity lung disease, coronary artery disease status post percutaneous coronary intervention (3x, last 4 years ago), sick sinus status post pacemaker, and implantable cardiac monitor for syncope.
Patient Data
The dual lead left-sided cardiac pacemaker and its right atrial and right ventricular leads are in appropriate position.
The implantable cardiac monitor/implantable loop recorder is seen projecting over the left mid-lung zone on this exam with mild rotation to the left.
Mild irregular bibasilar opacities are likely fibro-atelectasis.
No acute cardio-pulmonary process.
Case Discussion
The implantable loop recorder (ILR), also known as an insertable cardiac monitor (ICM), diagnoses heart rhythm disorders as a subcutaneous device. The ILR investigates unexplained recurrent syncope with uncertain diagnosis and reveal possible relationships with cardiac arrhythmias. In addition, the ILR is equipped with algorithms to detect atrial fibrillation episodes.
