Incisional hernia

Diagnosis almost certain

Presentation

Abdominal pain and vomiting for the last 1 day. Prior history of laparotomy and ? secondary suturing about 30 years back.

Patient Data

Age: 70 years
ultrasound

Three adjacent hernia defect in infra-umbilical anterior abdominal wall.

Non-reducible small bowel hernia is noted through one of the defect. Herniated loop was dilated without wall odema. Wall  vascularity was presennt.

Non-reducible fat hernia is noted through the other two defect in infra-umbilical anterior abdominal wall.

Case Discussion

Findings favor multiple incisional hernia defect. Small bowel hernia through one of the defect resulting in small bowel ostruction.

Laparotomy was done. Herniated loop was viable.
