Presentation
Dyspnea with wheezing. Abnormal aortic arch on chest x-ray.
Patient Data
there is a right-sided aortic arch with the left brachiocephalic trunk arising first, followed by the right common carotid and right subclavian arteries (mirror image branching)
pulmonary consolidation of the right segment 2 with air-bronchogram
developmental outpouching of the descending aorta (descending aortic diverticulum) suggestive of an atretic distal left arch
moderate tracheal stenosis (due to a vascular ring as there is a mirror branching on both sides)
Case Discussion
The clinical presentation and tracheal stenosis are suggestive of a symptomatic vascular ring due to an incomplete double aortic arch with atresia of the distal left arch 1.
