Presentation

Early pregnancy case presented with vaginal bleeding and leakage of liquor then-recent gush of fluid.

Patient Data

Age: 40 years
Gender: Female

Pelvic ultrasound after admission showing intrauterine fetus about 15 weeks gestation with breech presentation, severe oligohydramnios, and fetal lower limbs in the cervical canal.

Follow-up pelvic ultrasound showing a collapsed baby in the vagina with continued cardiac pulsations noted along with an intrauterine retained placenta.

Intraoperative trans-abdominal pelvic ultrasound after the complete evacuation of the retained placenta and uterine curettage.

Case Discussion

History of recurrent spontaneous miscarriages, as her previous pregnancies, end in the same way with a rupture of membranes in the 4th-month of pregnancy.

An inevitable miscarriage is an early pregnancy abortion with dilatation of the cervical canal that can't be prevented.

This case passed through different phases of abortion started with threatened abortion with liquor leakage then cervical dilatation and inevitable abortion and ended with a retained placenta as incomplete abortion which further needed a D&C.
