Infantile subdural hygroma following vacuum delivery

Presentation

Infant with irritability, head circumference 99.9%

Patient Data

Age: 2 months
Gender: Male

Large bilateral symmetric subdural hygroma with density thicker than that of CSF. No evidence of brain compression. 

Case Discussion

History-uncomplicated vacuum delivery, head circumference 65%. Gradually increasing head circumferences noted at pediatric visits in the first several weeks. Irritability age 2 months

Exam-Head circumference 99.9%

CT/MRI-no scalp or skull abnormalities, no hemorrhage, normal diffusion study

Subdural drainage by the neurosurgeon drained 40mL of watery fluid

The history of vacuum delivery followed by gradually increasing head circumference suggests that this finding is a complication of the delivery.
