Infected first branchial cleft cyst

Presentation

Inflammatory swelling in the left parotid region.

Patient Data

Age: 55 years
Gender: Male

A well-defined partially septated cystic mass located superficial to the left parotid gland. It appears hypointense on T1, inhomogeneously hyperintense on T2 with a thick enhanced wall on postcontrast sequences and restricted diffusion on DWI/ADC.

No obvious communication with the left external auditory canal.

Case Discussion

The MRI features are highly suggestive of infected first branchial cleft cyst.

The patient went on to have a complete surgical resection of the cystic mass with a histopathological exam that confirmed the diagnosis of branchial cleft cyst.
