Infected lymphocele post radical prostatectomy

Fevers. Radical prostatectomy 2-3 months prior.

Patient Data

Age: 70
Gender: Male

Multiple clips within the pelvis in keeping with radical prostectomy.
There are bilateral near water attenuation collections tracking along the iliac vessels, surrounded by surgical clips, the right measuring 9.9 x 5.3 x 8.9 cm, and the left measuring 2.5 x 3.8 x 3.8 cm.
The right-sided collection demonstrates wall thickening and apparent rim enhancement. Contrast is seen within the urinary bladder on delayed phase imaging.

Case Discussion

Findings are compatible with bilateral post-operative lymphoceles, the right much larger than the left. The wall thickening and rim enhancement would not be expected in a simple lymphocele, and is strongly suggestive of infection, which would explain the patient's fevers.
