Presentation

Painful left neck swelling

Patient Data

Age: 35 years
Gender: Female

ultrasound

Examination of the left side of the neck revealed a well-defined cystic lesion with thick wall and mixed echogenicity content.

Multiple locoregional cervical lymph nodes are noted.

Left-sided cervical cystic lesion with a well-defined wall and fluid content that shows dependent levelling on T2. Multiple cervical lymph nodes are noted.

Aspiration done under US guidance.

Case Discussion

The radiological appearance is possible for an infected second branchial cleft cyst. The presence of bilateral cervical lymphadenopathy, with some of them showing necrosis raise the possibility of metastatic cervical lymphadenopathy. The result of the US-guided needle aspiration and follow up is not available.
