Dyspnea and chest pain. The patient is heavy smoker

Age: 45 years old
Gender: Male

The inferior accessory fissure is normal variant which separate the medial basal segment from the other basal segments of right lower lobe. The patient also has centrilobular emphysematous changes at upper lobes of both lungs.

Diagnosis: Inferior accessory fissure
