This lesion in the left breast was considered to possibly be an intramammary lymph node but note the density of the lesion and the slightly spiculated margins.

On magnification the lesion is not well defined.

The ultrasound shows a small spiculated solid lesion that was malignant on biopsy.
Case Discussion
You cannot assume all lesions that are small and in the upper outer quadrant of the breast are in fact lymph nodes. Lymph nodes are low density, smoothly circumscribed and have a fatty hilum.
This lesion was a 5 mm malignancy.
