Presentation

Perineal pain and tender vaginal swelling

Patient Data

Age: 40 years
Gender: Female

Posterior and left lateral periurethral small cystic lesions eliciting low T1 and high T2 signal with diffusion restriction of some of them and post contrast enhancement.

Impression: inflamed urethral diverticula.

Case Discussion

Urethral diverticula are located in the middle third of the urethra and involve the posterolateral wall.

Clinically, they are felt as tender cystic lesions located in the anterior vaginal wall. Patients with urethral diverticulum may have multiple or compound diverticula.

Diverticula usually arise from the paraurethral glands, likely resulting from infection and obstruction of these glands with subsequent abscess formation which eventually ruptures into the urethral lumen.

MRI is considered a useful imaging modality for the diagnosis of urethral diverticula.
