Asymptomatic. Incidental finding on routine dental imaging.

Age: 18 Years

There is a well-defined, corticated, hydraulic, radiolucency noted associated with the mandibular right third molar and superimposed over the roots and furcation region. The lesion extends from the furcation region to the anterior ramus of the mandible just distal to the tooth and supero-inferiorly, it extends from the alveolar crest to the superior cortex of the inferior alveolar canal.

No radiographic signs of cortical interruption, expansion or tooth displacement were noted in this study.

Case Discussion

Unilocular radiolucency in the mandible associated with a third molar in the region of the furcation region is suggestive of a benign, odontogenic, cystic process and differential diagnosis will include inflammatory collateral cyst such as buccal bifurcation or paradental cysts and less likely a dentigerous cyst.

Excisional biopsy was performed and microscopic examination revealed a non-keratinized, stratified, squamous lining epithelium with fibrous connective tissue. The lining epithelium composed of flattened squamous to cuboidal cells. Fragments of bone and islands of inactive odontogenic epithelium noted in the specimen.
