There is a right inguinoscrotal hernia through the inguinal canal, medial to the inferior epigastric vessels, containing mesenteric fat and vessels, and small bowel loops with no signs of small bowel obstruction or necrosis.
A small direct inguinal hernia is also noted on the left.
Case Discussion
Direct inguinal hernias are the result of a weakness of the posterior wall of the inguinal canal medial to the inferior epigastric vessels through the Hesselbach's triangle, They are generally acquired and increase in incidence with age.
