Presentation

Painless subcutaneous swelling

Patient Data

Age: 25 years
Gender: Male

There is a well-defined, low attenuation, non-enhancing mass lesion in the left medial canthus extraconal location with scalloping of the adjacent nasal bone, measuring approximately 2.3 x 2.7 x 2.7 cm in size. The lesion shows fat attenuation (-160 HU). No evidence of calcification. No intraorbital or intracranial extension.

Case Discussion

CT features are consistent with an internal angular dermoid cyst. It usually appears as a well-defined cystic lesion at extraconal location with smooth wall and fat content. The most common location is at the frontozygomatic suture (80%) followed by the nasal aspect of upper orbit at the frontoethmoidal suture. Orbital dermoid cysts could be diagnosed if the fatty components are found within it.
