Internal carotid artery aneurysm

Presentation

Patient known to have left ICA aneurysm presenting through ER with severe headache.

Patient Data

Age: 60 years
Gender: Female

Brain

There is around two cm left internal carotid artery saccular aneurysm with associated acute intraventricular and subarachnoid hemorrhage which is also seen extending supra and infratentorially.

Mass effect is seen with partial effacement of sulci.

Faint bilateral cortical diffusion restriction is also seen indicating acute hypoxia-ischemic changes.

Findings of chronic ischemic changes are also seen.

Case Discussion

This is a 60-year-old lady known to have left ICA aneurysm presenting with sudden onset severe headache through A&E department. Brain MRI was done demonstrated additional evidence of recent intraventricular and subarachnoid hemorrhage.
