Age: 70 years
Gender: Male

Cervical segment left internal carotid dissection with the true and false lumen seen on the TOF MRA. No crescent sign on T1FS sequences. Restricted diffusion in the left internal capsule.

Case Discussion

This case is atypical for an internal carotid dissection because the typical dissection includes occlusion and high signal T1 crescent sign within the wall of the vessel (hematoma). The only feature fitting carotid dissection is evidence of cerebral ischemia. A double barrel lumen as this case is a rare presentation possibly due to carotid dysplasia.
