CT brain

Hyperdense internal cerebral veins, vein of Galen, and straight sinus are noted. Note the bilateral thalamic edema  most marked on the right.  A region of low attenuation in the right parietal region is old (seen on previous scans - not shown). 

CT venogram

CT venogram confirms absence of enhancement of the internal cerebral veins, vein of Galen and straight sinus.

CT brain: 1 day later

Single image through the brain obtained the next day demonstrates prominent low density change involving both thalami and basal ganglia. Hyperdense viens are again seen. 

CT brain: 2 days later

A right sided thalamic venous hemorrhage has developed. 

Case Discussion

Internal cerebral vein thrombosis can be difficult to identify if the reporting radiologist is not familiar with the condition and should be suspected in cases of bilateral thalamic edema.
