Interstitial edematous pancreatitis

Presentation

Upper abdominal pain for four days, becoming more severe within the last two days, dull aching and radiating to the back. Serum amylase was 3194 U/L.

Patient Data

Age: 40 years
Gender: Female

Edematous entire pancreas accompanied by moderate fat stranding and peripancreatic fluid. No necrotic areas, vascular insults, or peri-pancreatic localized collections.

Mild intra-peritoneal free fluid.

Bilateral minimal pleural effusions with basal pulmonary aspirations. 

Two isodense gallbladder stones. Average-caliber common bile duct.

Diagnosis: acute interstitial pancreatitis, moderate in severity according to modified CTSI (4 points: 2 for pancreatic inflammation and 2 for ascites & pleural effusion), in a patient with calcular gallbladder.

Case Discussion

Two of the following three criteria are required for the diagnosis:

  • acute onset of persistent, severe epigastric pain (i.e. pain consistent with acute pancreatitis)

  • lipase/amylase elevation >3 times the upper limit of normal

  • characteristic imaging features on contrast-enhanced CT, MRI, or ultrasound
