Intertrochanteric fracture - subtle

Case contributed by Bálint Botz
Diagnosis certain

Presentation

Sustained a fall.

Patient Data

Age: 75 years
Gender: Male

While the AP pelvis and hip radiographs show no gross abnormality, the minimally displaced fracture extending from the greater trochanter is clearly visible in the oblique view. With this information in mind the fracture can be discerned in the AP views as well (see zoomed in key images).

Case Discussion

1. Minimally displaced intertrochanteric fractures are often better (or only) visible in the oblique views.

2. Pelvis AP radiographs alone can not be used to rule out this type of fracture.
