Acute diverticulitis. The small bowel is right sided, with predominantly left sided colon. The cecum is in the right upper quadrant. There are features of reversed rotation, with the transverse colon passing posterior to the duodenum, and the superior mesenteric artery and vein. Midgut volvulus is present, with reversed relationship of SMA and SMV, and "swirled" mesentery. No evidence of obstruction or vascular compromised.
