Intestinal obstruction due to adhesions

Presentation

Vomiting and constipation for the past three days, with a history of appendectomy.

Patient Data

Age: 70 years
Gender: Male

Dilated, fluid-filled bowel loops involving the small bowel, cecum, and ascending colon up to the hepatic flexure where there is a transition point between the dilated proximal part and the collapsed distal part.

Incidental lipomatosis of the ileocecal valve.

Right lower lung consolidation and cardiomegaly.

Case Discussion

The finding in this case, especially with a history of previous abdominal operation for an appendectomy, is most likely intestinal obstruction due to adhesions.

The patient underwent a surgical laparotomy, which revealed an adhesive segment that was removed, and an end-to-end anastomosis was performed.

Lipomatosis of the ileocecal valve is a common incidental finding with variable sizes and is usually of no clinical significance.
