Intra-articular foreign body in proximal interphalangeal joint

Case contributed by Maulik S Patel
Diagnosis almost certain

Presentation

Swelling of the middle finger for 2 -3 weeks.

Patient Data

Age: 55 years
Gender: Male

A linear echogenic focus (1.4 mm size) is noted in the proximal interphalangeal joint of middle finger with associated palmar recess effusion. It is not mobile with finger movement or with traction of the finger.

Case Discussion

The patient (a farmer by occupation) could not recollect any local penetrating trauma. However, farmers are prone to wood splinter injury which may be trivial and the patient may not confirm it.

Proximal interphalangeal joint effusion was suspected on examination and the patient was referred to ultrasound exam to rule out or localize foreign body.

Local skin marking was done at site of foreign body and it was removed surgically. There was no pus in the joint.
