Presentation

Painless infrapatellar swelling with a palpable soft fluctuant mass.

Patient Data

Age: 40 years
Gender: Female
mri

Well-encapsulated infrapatellar intra-articular mass measuring 45 x 35 x 20 mm of high signal on T1 and T2 attenuated on fat-suppressed sequences, surrounded by a thin rim of synovial fluid indicating its intra-articular location well-visualized on axial PD fat sat sequence. A mass effect is noted on the Hoffa fat pad. Minimal joint effusion.

Normal appearance of the cruciate and collateral ligaments as well as the menisci.

On imaging, the main differential diagnosis is a Lipoma arborescens which usually shows a frond-like appearance.
