X-ray shows a large homogeneous, well defined mass projected over the cardiac silhouette. There is no distortion of the cardiac contour or loss of the heart borders. Appearances likely reflect a large posterior mediastinal mass but there is no rib splaying or vertebral abnormality.

Subsequent CT shows that the mass is in fact an intra-thoracic kidney with the vascular pedicle extending from the abdomen.
